Rules & Regulations for Certified Medicaid Providers in Clients’ Homes

To ensure professionalism, compliance, and client safety, all Certified Medicaid Providers working with LHE Home Care Unlimited must follow these rules and regulations while in a client’s home.



1. Professional Conduct & Ethical Standards
• Providers must always act professionally and respectfully while in a client’s home.
• Maintain appropriate boundaries—personal relationships outside of care services are prohibited.
• No discrimination based on race, gender, religion, or disability.
• No verbal, physical, or emotional abuse will be tolerated.

2. Compliance with Medicaid Regulations
• Follow all Medicaid policies and procedures while providing care.
• No falsification of records—all services must be accurately documented and reported.
• No unauthorized services—only perform Medicaid-approved tasks in the care plan.
• No accepting cash, gifts, or personal favors from clients or family members.

3. Client Safety & Care Standards
• Follow the individualized care plan for each client as prescribed.
• Providers cannot administer medications unless they are licensed to do so.
• Report any signs of abuse, neglect, or medical concerns to the agency immediately.
• Keep the client’s home environment safe and hazard-free.

4. Documentation & Reporting
• Maintain accurate daily logs of all care provided.
• Report changes in the client’s condition to the agency and/or case manager.
• No fraudulent billing—only bill for actual services provided.
• Any incidents, injuries, or emergencies must be reported immediately.

5. Confidentiality & HIPAA Compliance
• Maintain client privacy—do not share personal health information (PHI).
• Securely store and handle all client documents and records.
• No taking photos or recording clients unless required for documentation with agency approval.
• No discussing client information with unauthorized individuals.

6. Timekeeping & Attendance
• Arrive on time and complete the full scheduled shift.
• Clock in and out accurately using the agency’s approved system.
• No unauthorized overtime or schedule changes without agency approval.
• Call the agency at least 24 hours in advance if unable to report for a shift.

7. Personal Conduct & Prohibited Behaviors
• No smoking, alcohol, or drug use in or near the client’s home.
• No use of the client’s phone, TV, or personal items without permission.
• No bringing unauthorized visitors or family members to the client’s home.
• No sleeping during shifts unless authorized for overnight care.

8. Termination & Disciplinary Actions

Failure to comply with these regulations may result in:
• Verbal or written warnings
• Suspension or immediate termination
• Legal action for fraud, abuse, or HIPAA violations
